Request an Appointment

Prior to requesting an appointment, please note the following:

  1. When booking an appointment online, you are making a REQUEST for an appointment.  Do not consider your appointment booked until it has been confirmed by me, at which point you will receive a notification indicating the confirmation.
  2. If you are a new client you will be required to fill out your intake forms prior to your appointment being confirmed.  I will send you the required forms when I receive your request.  Your appointment will not be confirmed until these forms have been completed.
  3. If you are covered by insurance you must provide your insurance information when submitting your intake forms.  I will need to validate your coverage prior to confirming your appointment.  Please allow for at least 3 business days to have your insurance validated.
